Here at West Sussex County Council (WSCC), what we do changes lives. We are looking for care and support brokers to join our combined placement and supply team! In this exciting opportunity, you will be at the forefront of helping our clients access appropriate and effective care and support services. You will report directly to the role of Placement and Supply Manager (or Deputy Manager) and provide care planning, supply and procurement functions for people in need of care and support who may be discharged from the hospital. hospital or those with long-term care needs.

Mental Health and Lifelong Services team – based in Bridge House, Worthing.

  • You will primarily be required to assist people with lifelong or learning disabilities and those with mental health needs to access services as part of a new team and service being developed.

Senior Residential Team – based in Bridge House, Worthing

  • You will primarily need to help seniors plan and access nursing homes and nursing homes.

North Team for Non-Residential Elderly – based in County Hall North, Horsham

  • You will primarily be required to help seniors in our North region plan and access non-residential services such as in-home care or day care

Southern Non-Residential Elderly Team – based in Bridge House, Worthing

  • You will primarily be required to help seniors in our North region plan and access non-residential services such as in-home care or day care

As a Care and Support Broker, you will develop in-depth knowledge and specialist skills within customer groups and service types. You will be adaptable and skilled in helping the team respond to changing demands and pressures. You will work closely with clients, their families or representatives to plan effective care and support services. You will liaise regularly with other WSCC departments and those of other stakeholders, including NHS departments and healthcare providers.

Experience and skills

Core Skills:

  1. Able to support the planning and organization of client-centred and strengths-based services for people with care and support needs.
  2. Able to accurately relay information to service providers and health and social care colleagues to enable rapid search for appropriate services.
  3. Able to organize/prioritize work, work independently and work under pressure and to tight deadlines.
  4. Able to interpret incoming information to identify risks and communicate backup and QoS issues through appropriate channels.
  5. Excellent written, verbal, influencing and negotiation skills.
  6. Strong and accurate computer and keyboard skills.

Qualifications and/or experience:

  • NVQ Level 3 in a relevant subject, or equivalent qualification or experience.
  • GCSE English and Maths, or equivalent qualification or experience working at that level of literacy and numeracy.
  • Good up-to-date working knowledge of legislation and organizational policies relevant to the specialist area of ​​the post, e.g. Care Act (2014), Discharge Guidelines 2020, Mental Health Act (1983) , etc
  • Experience monitoring data and information, including financial equipment and using a range of computer and data systems.
  • Good understanding of office systems and practices related to performing a health or social care role.
  • Experience influencing and negotiating with individuals, stakeholders and service providers – Desirable
